“We hope that by having a grand jury return an indictment in this case it will bring us one step closer to a trial and achieving justice.

“We take our accountability seriously, the rest of the system is not doing that

“Those are mistakes and those are our mistakes, that is part of what is being investigated and that is part of what we've apologised for
